- Acceptance of the Invitation conditions
In accepting this invitation to register and use aSAP, you acknowledge Accessable’s privacy requirements regarding accessing and sharing information from the customer record. Specifically, this requires you to abide by the requirements of the Privacy Act 1993 and Health Information Privacy Code 1994; and also to ensure customers have information about our management of their health information and have consented for you to enter or access their record on our system.
- Customer Information – acknowledging your responsibility
As Accessable does not have direct access to the customer, we rely on you to provide our Privacy statement to them. In registering for and accessing this system, you acknowledge this responsibility, and agree to ensure customers are aware of Accessable’s privacy statement and their rights under the Health Information Privacy Code 1994. This is available on our website.
By entering the aSAP portal, you acknowledge you have informed consent from the customer(s) for the provision of and/or access to the information in the health record.
- Confidentiality of Information – Non-Disclosure Agreement
Accessable has provided a significant amount of confidential information within aSAP including but not limited to supplier catalogues of equipment with pricing. As a condition of accepting this invitation and use of aSAP, you the user agree not to disclose any confidential information to third parties who are not expressly permitted to view such information unless you are required to by NZ law. You agree to use the information only for the purposes for which it has been disclosed to you. You agree not to reproduce or store the confidential information except for single use relating to applications or service requests. You will also keep confidential Accessable’s aSAP platform functionality and processes.
You agree not to share your user profile or login details aSAP with anyone else.
- General T&Cs
Any breach of the Privacy and Confidentiality agreement may result in the user having their access to aSAP withdrawn.
This agreement remains in force even after you have ceased having access to aSAP and you hereby agree to remain bound by the confidentiality and non-disclosure agreement terms and conditions.

Need an aSAP Login?
If you are an accredited assessor with Disability Support Services (the Ministry) but are not registered for aSAP, complete and submit the aSAP registration identification form here:

Becoming an accredited assessor (Ministry)
If you are not an accredited assessor, you will not be able to apply for Disability Support Services (Ministry) funding to provide solutions for your customers. You first need to be registered as an accredited assessor. Find out more here:

Service Request tips and tricks
Get some tips and tricks on creating a Service Request
Here are some tips to make submitting your applications a little easier:
- Photos can sometimes be too large to upload into aSAP. Put them all into one PowerPoint and they will upload with no problem. (It’s also easier for us).
- When entering items into sections in an advice request, make sure you click the ‘ADD’ button (on the left of the field). If you don’t do this, it doesn’t save.
- Don’t forget to upload your P1 form for each service request that goes through the EMS Portal. If it isn’t there, the service request will be withdrawn.
- If your service request says ‘withdrawn to edit’ that means we have withdrawn it as not all required information or documents are there. It is sitting with you and you need to make the changes and re submit.
- If your service request says ‘waiting for information’ it means we are waiting on you to submit some information. If you aren’t sure what that is, contact our customer support team.
- You can upload multiple files at a time by holding down the ctrl or shift key when selecting them from file manager.
- There's an arrow to the right of each section heading. You can click this to hide that section once you've completed to reduce the information you can see on the screen
Documents required in a Service Request
Find out what documents you need to include in a Service Request
- P1 form. If you have gone through the EMS Portal, you will have included a Priority 1 form. This needs to be uploaded to the Service Request.
Equpment
- Quote. If you are requesting a non-catalogue item in your Service Request, you must have a current quote. When marking a trial successful and the quote is older than it is valid for, you must get a new quote. (If trials are completed within the mandatory 20 days, the quote will still be valid and you won't need a new one).
- Spec forms. If you are requesting wheelchairs or commodes that are highly configurable, you must complete a specification form.
Housing
- Information and property owner form
- Enduring Power of Attorney (if relevant)
- Quote from the builder.
- Spec forms. Mandatory for low rise lift and level access shower service requests.
